RE: PARENT/GUARDIAN - REQUEST FOR PERMISSION TO CONDUCT A CASE STUDY
My name is ______________________ and I am a childhood education foundation phase student at the University of Johannesburg. One of the course requirements for this semester is to do a case study for the different aspects of human development. I hereby request your permission to conduct this case study on your daughter/son, ____________________________.

Your child’s participation in this research process is voluntary and she/he may choose to withdraw from this process at any time. If she/he chooses to not participate or withdraw from the research process, there will be no penalty. Furthermore, written consent will be obtained from your daughter/son as well.
For the purposes of anonymity and confidentiality the names of the school and your child will not be mentioned throughout the data and findings of the case study. Pseudonyms will be used in the writing of the final assignment. The collected data will be for this case study only, and apart from myself, only the lecturer will have access to the data. She will treat this information with the strictest of confidentiality. Should you so require, I will share the findings with you once my assignment has been completed.
