This is for your kind information that I, Smt. Arati Mazumder, W/O Lt. Manoranjan Mazumder, Ex-R/TRC,   had been   issued one Family Identity Card ( For Complimentary Pass ) bearing No. 2792 dated 09.09.2002 and on 09.12.2011 on the way to your office for renewal of the   Family Identity Card, I lost the same, hence, I lodged a missing dairy to the Office of the Officer-in-Charge of Police, NJP Out Post, NJP vide GDE NO. 330/11 dated 09.12.2011 (receipt copy enclosed) .   As I intend to make a trip in near future, I need a Family Identity Card ( For Complimentary Pass ).

You are requested to please issue a New or Duplicate Family Identity Card (For Complimentary Pass) as early as possible to enable me to avail of the facility.

It may be mentioned here that there is no survivor who fully or partially depended upon me.
